As an employee who was offered health coverage by MCH Transportation during the prior calendar year, you are entitled to a Form 1095-C. This document serves as proof of health insurance coverage for your tax records.

To reduce paper waste, MCH Transportation is providing this form upon request. You may request a free copy of your prior calendar year Form 1095-C by any of the following methods:
